    Why National Minority Health Month Matters in Home Care

    Health is a very broad category and difficult to measure comprehensively, but across most metrics a clear pattern emerges: racial and ethnic minorities consistently suffer from worse health outcomes, and this also represents itself as health disparities in minority home care. The factors that affect health outcomes in this way are called the social determinants of health (SDOH), defined by the US Department of Health and Human Services as “the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.”

    Many negative SDOHs affect minority communities disproportionately, such as food insecurity and education and housing inequality.

    From The US Playbook to Address Social Determinants of Health: “…food insecurity has been associated with a 15% increased risk of having a chronic illness, twice the odds of having mental health issues in mothers and behavioral issues in children, and 58% increased risk of death from any cause.”

    Minority borrowers suffer an increased likelihood of mortgage rejection and higher mortgage rates and fees, which directly impacts the area and condition of the places they live. Lower-income housing is often disproportionately exposed to transportation and industrial pollution, which can add additional health burdens.

    Education (also impacted by the area in which you live) is a critical component to making informed decisions about health and medical treatment, not to mention finding jobs that provide enough pay for adequate health care. According to the National Institutes of Health, inequalities in education have actually caused health and longevity to deteriorate over time among those with less education, exacerbating the already widening health gap between economic and racial groups.

    As these SDOHs and the ways they interact are studied further, we aim to do our best to provide health education, support, and culturally competent healthcare to all who need it. This April, we celebrate National Minority Health Month, raise minority health awareness, improve health equity for seniors of all cultures, and look forward to a brighter, healthier future for everyone.

    Care Advantage is proud to share the impact of a recent collaboration focused on reducing avoidable emergency department (ED) utilization. By partnering with a managed care organization (MCO), we set out to better understand patterns of ED use and identify opportunities to deliver more proactive, coordinated care.

    Through targeted interventions—including enhanced care coordination, patient education, and improved access to preventative services—we were able to address many of the underlying drivers of avoidable ED visits and reduce hospital readmissions for members. The results have been both meaningful and measurable.

    For example, in one recent collaboration with an MCO, we saw a 37% reduction in the number of emergency department visits per member compared to baseline. This significant decrease highlights the value of a more integrated, patient-centered, and value-based approach to home-based personal care services.

    Reducing unnecessary ED utilization means patients are receiving the right care, at the right time, in the right setting. It also alleviates strain on emergency services and contributes to a more efficient healthcare system overall.

    Long-Term Care Insurance almost seems like a buzz word these days. While many of these policies have been around for 30 or more years, they are being talked about now more than ever. With people living longer, and with most people wanting to age in place at home, it is a pretty safe bet that personal care in the home will be needed. Long-Term Care Insurance is a supplemental insurance that covers most aspects of personal care in the home, ensuring that other retirement savings go toward their intended goals and not to the costs of care.

    While Care Advantage has worked with Long-Term Care Insurances for years, clients worked directly with the insurance provider to line everything up. We have decided to take the burden of figuring out Long-Term Care Insurance OFF our clients and handle this ourselves! 

    With the hiring of an exceptional LTC Specialist, we will now be able to take on Assignment of Benefits for our LTC clients, and provide a seamless, stress-free process from the start. Clients won’t have to worry about who to call, what to ask, or how to get reimbursed. We will do the paperwork to facilitate communication between the LTC Insurance Provider and our agency, allowing our highly experienced clinical team to provide care notes in real time. Best of all, we will be all set to bill the insurance company directly without the client having to wait for reimbursement!

    HOW TO HIRE AN IN-HOME CAREGIVER

    It’s official: You’ve decided to hire an in-home caregiver for yourself or a loved one. That is a great weight off your mind, and you’re looking forward to getting the extra support you need. 

    Still, the practicalities seem daunting. How do you find the right person — or team of people — to meet your requirements? 

    Here’s our three-step plan on how to choose an in-home caregiver. 

    ●    Step 1: Work out when you need your caregiver to start 

    Some needs can’t wait, so deciding when to hire an in-home caregiver for an elderly loved one often comes first. For example, if a previously mobile senior suffers an injury, or is discharged from the hospital to recuperate after surgery, they may suddenly go from needing no care at all to requiring round-the-clock supervision. 

    If you must have help right away, we recommend you go through an agency. Hiring the right private caregiver takes time and cutting any corners could put you or your loved one at risk. Care Advantage promises to provide you with the help you need within 24 hours of your request. 

    ●     Step 2: Define what you’re looking for  

    Try to imagine what the perfect care plan for you or your loved one would look like. Do you want someone for companionship and grocery runs, or for 24/7assistance from a nursing provider — for instance, someone who can administer IV drugs and change dressings? 

    Next, to hire a private caregiver, turn this wish list into a job description, then advertise for candidates. However conscientiously you do this, there’s always a chance you’ll overlook something — like forgetting to mention your loved one’s menagerie and that caregivers with pet allergies need not apply.

    ○    Finding last-minute cover for caregivers who call in sick  

    If you go through an agency, you will pay somewhat more but do far, far less — including none of the above.
