The Home-Based Care Momentum Awards, presented by AlayaCare, recognize outstanding individuals who are elevating home-based care every day, from the front lines to the boardroom. AlayaCare believes technology is only as powerful as the people who use it, and this award exists to celebrate exactly that: the late-night visits, the quiet acts of advocacy, and the everyday problem-solving that moves this industry forward.
Our 2026 Winners
Frontline Care Recognizing exceptional frontline caregivers and clinical staff
Brianna Allen, Caregiver During an icy weather event in early 2026, when roads were dangerous and communities were advised to stay home, Brianna continued to show up for her client. On two of those days, when driving simply was not possible, she walked. Her client reached out to our Director of Clinical Services specifically to express his gratitude. Brianna is a living example of our I CARE values.

Dagmaly Santiago, Caregiver Dagmaly has been with Care Advantage, Inc. for over five years and drives an hour each morning to make sure her client receives the care they need. She shows up without fail, maintains excellent EVV compliance, and has built the kind of relationship with her client’s family that speaks for itself. Her reliability and the warmth of her care have made her someone this family counts on completely.

Daniel Flaherty, RN, Home Care Supervisor Dan leads with both clinical excellence and genuine care for the people he serves. His clinical documentation is nearly flawless as he takes time to explain care plans and client handbooks thoroughly. He also has built strong, trusting relationships across his caseload. He is vocal in team meetings, a strong advocate for Care Advantage, Inc., and by every measure, the epitome of our I CARE values.

Operational Excellence For outstanding operational performance and efficiency
Shannon Loving, RN, Home Care Supervisor Shannon does not see this as an 8-to-5 job, and it shows. She is patient, proactive, and consistently puts her clients’ needs first. When she sees a gap, she does not wait for someone else to fill it. She brings solutions forward and follows through. Her clients are better cared for because of her advocacy, and her team is stronger because of her example.

Impact and Innovation For groundbreaking innovation and measurable impact
Chelsey Ammons, RN, Senior Vice President of Clinical Operations With nearly a decade of leadership experience in home care, Chelsey has built a reputation as a strong advocate for both patients and Caregivers. She has been instrumental in shaping value-based care initiatives that have positioned Care Advantage, Inc. as a partner of health systems seeking better outcomes. A registered nurse with a deep understanding of both clinical practice and human behavior, Chelsey brings a perspective that is rare and genuinely impactful.
