In accordance with the Affordable Care Act (ACA), the Company, as an Applicable Large Employer (ALE), provides Form 1095-C to individuals who were eligible for
coverage during the applicable tax year.
Form 1095-C is now available electronically through the employee online portal (ADP) for both current and former employees.
Form 1095-C will not be mailed this year. Individuals may access, view, and download their form online at their convenience.
Paper copies are available upon request. Any individual who would like to receive a paper copy of Form 1095-C, you may request one by contacting:

Upon receipt of your request, a copy of your Form 1095 will be furnished within 30 days.
This notice will remain available on this website through October 15 of the year following the applicable coverage year.
