Meet Brittany Schwemmer, VP of Human Resources
Brittany Schwemmer brings nearly 20 years of human resources expertise to Care Advantage, Inc., and that experience runs deep. As our new VP of Human Resources, Brittany leads HR strategy, operations, and organizational development across our multi-state footprint, with a track record of building the kind of workplace cultures where people genuinely thrive.
Her background spans employee relations, compliance, compensation and benefits, talent management, HR technology, and M&A integration. But what sets Brittany apart isn’t just her résumé — it’s her approach. She’s known for pairing big-picture strategic thinking with a hands-on, roll-up-your-sleeves style that gets things done. She’s passionate about building high-performing teams and creating workplaces where employees feel valued and supported.
For a company like Care Advantage, Inc. where our Caregivers are the heartbeat of everything we do, having a people-first leader like Brittany in our corner means a great deal.
A Little More About Brittany
Outside of work, Brittany is a true fitness enthusiast and a devoted animal lover, the proud mom of two dogs, a rescue mix and a Bernese Mountain dog. She’s also a certified Pure Barre instructor! When she’s not teaching Pure Barre or at the office, you’ll find her exploring the outdoors, spending time with friends and family, or unwinding with a good podcast.
Meet Gabriel Toban, Director of Data & AI
Gabriel Toban, PhD, joins Care Advantage, Inc. as our new Director of Data & AI, and we couldn’t be more excited about what he brings to our organization. With a background that spans physician practices, hospitals, insurance, healthcare consulting, enterprise business intelligence, and clinical research, Gabriel has spent his career turning complex healthcare environments into systems that make sense and make a difference.
His specialty? Taking complicated data and transforming it into clear, trusted tools that help care teams work smarter. From large-scale reporting transformations to AI-enabled analytics solutions, Gabriel has a gift for making technology feel less like a burden and more like a superpower. He’s deeply passionate about using data not just to improve efficiency, but to support better care outcomes for real people.
At Care Advantage, Inc., where delivering high-quality, compassionate care is our top priority; Gabriel’s work will help us get there faster, smarter, and with greater confidence.
A Little More About Gabriel
When he’s not transforming data systems, Gabriel is kept wonderfully busy at home. He and his wife are proud parents to three daughters, ages 7, 8, and 13, and one very loved 2-year-old pup. The Toban family calls East Nashville home, where they keep busy with various family activities. Outside of family time, Gabriel unwinds with kickboxing. And if his day job in AI wasn’t impressive enough, he holds a PhD in Computational and Data Science with a focus in Neuroscience.
